San Francisco 49ers wide receiver Ricky Pearsall is swiftly starting to shift toward being yet another one of general manager John Lynch's draft whiffs.
It's not exactly Pearsall's fault, though, as it's certainly hard to hold many of the injuries he's had against him (primarily being shot in a robbery attempt on the eve of his rookie season).
Yet injuries have been as big a theme for the former Florida Gator since he went pro, and they're seeping through again into his third year at the NFL level.
According to a training camp report from ESPN's Nick Wagoner, Pearsall is dealing with swelling related to the PCL injury in his knee that largely influenced why he played in no more than nine games last season:

Tacking on Wagoner citing "all options" and "concern," and it's absolutely problematic for the 2024 first-round draftee's future, both in the immediate and long-term pictures.
Ricky Pearsall potentially faces missing all of 2026 campaign
One of those options would include surgery, and according to Jeff Mueller, a physical therapy physician and analyst on Fantasy Points, this type of procedure could ultimately result in Pearsall missing all of 2026:

"For a PCL, if there’s still instability and they’ve already tried rehab, plus PRP, then surgery would likely be the answer," Mueller wrote on X. "That would cost him all of 2026."
For a player who already missed chunks of his previous two seasons with injuries, and with the understanding how crucial 2026 was supposed to be, this development would be absolutely brutal.
49ers may have little choice but to admit failure with Ricky Pearsall
When healthy, Pearsall showed plenty of flash and promise, both as a rookie and second-year pro. Those helped reinforce why San Francisco's selection of him toward the tail end of Round 1 back in 2024 was a good move.
But chronic knee issues are dangerously close to ultimately derailing whatever value Pearsall has to the offense.
Without diving too deep into long-term impacts of a medical procedure, there has to be concern Pearsall isn't quite the same after undergoing surgery and recovering from it. And it'd be a foregone conclusion the 49ers would pass on exercising the receiver's fifth-year option—a decision they'd have to make an offseason from now.
Possibly, Pearsall's surgery now could lead to a full recovery while also keeping his would-be asking price down on a future extension. But that's risky, and few teams would be willing to take such a risk without serious contingency plans in place.
And those contingencies typically involve replacement-level options, pointing at a reality where the Niners admit drafting Pearsall didn't come close to generating desired results.
That'd be as disappointing a development as possible.
