George Kittle 5-yard touchdown reception (No. 5)

With just 24 seconds remaining, the 49ers have a 4th-and-goal at the 5-yard line. The Niners line up with 11 personnel in a shotgun formation, with Brian Hoyer motioning wide receiver Trent Taylor across the formation prior to the snap.

After the ball is snapped, the Colts blitz a total of seven defenders in front of a Cover-0, leaving the middle of the field wide open. Kyle Shanahan calls the perfect play  — a quick pass to Kittle, who has separation thanks to Taylor running his route between the tight end and safety Malik Hooker.

Hooker does a good job of quickly closing the gap, but Kittle does a better job of elevating his legs away from the turf as he extends the ball into the end zone for a game-tying touchdown:
